Ticket image covers help you visualize your tickets. They are only visible in Board View:
Whenever you upload a picture to a ticket, the latest one becomes the ticket cover. You can change at any time which image is the cover one, or remove the cover altogether.
You can also disable this feature globally via the "options" menu.
